If T1DM makes your child unable to learn with the rest of his class (because he suffers from frequent hypoglycemia or frequent hyperglycemia, for example), he may need an individualized education program. This is a detailed program for his education based on the requirements of his diabetes as well as his special education needs. It's more of an education program than a health program, which is why I don't get into it extensively in this book. The need for this type of program is determined by parents and school officials working together. For a complete guide to the individualized education program and a program document template, visit www.ed.gov/parents/needs/ speced/iepguide/index.html#form.
