How will you treat the patient

It is necessary to surgically debride the ulcer, remove the callus and cover its surface with a bandage. Callus removal decreases the applied pressure on the ulcer area. A foot X-ray, focused on the ulcer area, is also necessary for ruling out obvious osteomyelitis. If there is foul odour, accompanying cellulitis or a dirty ulcer base, it is indicated to get a culture from the ulcer base, after surgical debridement, for identification of infection-causing bacteria and administration of the proper antibiotics.

As mentioned above, the corner-stone of ulcer treatment is the decrease of the applied pressure in the ulcer area, until complete healing is achieved.
