It has been more than 30 years since the first aldose reductase inhibitor (ARI) was tested in diabetic and galactosemic rats and found to control the polyol accumulation (1). Since then, a considerable number of ARIs have been tested in experimental and human diabetes. However, very few new information have become available since the last edition of this book, probably an indication that either interest in these compounds is waning down in the scientific community or that despite all the intensive efforts, the ideal compound that will offer satisfactory enzyme inhibition with minimal side-effect has not been discovered as yet. A thorough review of work on experimental diabetes would be out of the spirit of this chapter; however, more information is provided in chapters of this edition. The following chapter will focus on the results from clinical trials in diabetic neuropathy (2).
