How is the action of inhaled insulin influenced by asthma and acute respiratory tract infections

Individuals with asthma absorb less quantity of insulin from their lungs and thus need more units of inhaled insulin. However, studies with regard to asthma and acute respiratory infections are still rare and it is expected that after the widespread use of treatment with inhaled insulin more explicit evidence will become available. Moreover, the use of different types of insulin and different inhalation appliances render any firm conclusions from the few publications and studies premature.

What Is Asthma

During asthmatic episodes, or attacks, the body's airways (bronchi and bronchioles, which deliver air to the lungs) overreact to pollen, cat dander, cigarette smoke, cold air, or other stimuli. The airway wall muscles begin to spasm, narrowing the opening and allowing less air to reach the lungs. An inflammatory response results in a rapid thickening of mucus, which further narrows the airway. An asthma attack may begin with wheezing, coughing, or shortness of breath suffocation can occur in rare cases.

The Inflammation Syndrome Connection

Asthma usually begins when the immune system becomes sensitized to an allergen, which may be a food, tobacco, proteins from cockroaches, or dander from pets. The immune system responds whether the trigger is a real threat or not, releasing a variety of inflammatory molecules. An upper respiratory tract reaction may cause little more than a runny nose (rhinitis), but asthma develops when the bronchi or bronchioles (which branch out from the bronchi) start overreacting to inflammatory molecules. Vitamin E and Nasal Allergies

IgE is an antibody produced in excessive amounts by people with asthma, rhinitis, and hay fever. Fogarty reported in Lancet that higher vitamin E intake was associated with lower levels of IgE. Each 1-mg increase in vitamin E intake, up to 7 mg (about 11 IU), was related to a 5.2 percent reduction in IgE levels.

A new device called Rapid Mist is currently being tested that could deliver an aerosolized version of insulin through the mucus membranes of the cheeks, tongue, and throat. Early results indicate that Rapid Mist, which resembles an inhaler used by people with asthma, is identical to an insulin injection in its ability to lower blood glucose levels.

Comments on Inhaled Insulin

Smoking is a contraindication for inhaled insulin and inhaled insulin is not recommended in patients with asthma or chronic obstructive pulmonary disease. All candidates for inhaled insulin should have their lung function checked before and after 6 months and then every year. If lung function has declined more than 20 or by more than 500 ml from baseline, inhaled insulin should be discontinued. The long-term effect of inhaled insulin in the human lung and on neoplastic lung tissue is unknown. Pulmonary insulin is much more expensive than human insulin and still needs to be compared with the rapid-acting insulin analogues. In the UK, the NICE institute recommends that inhaled insulin should be only prescribed by diabetes specialists for patients with needle phobia or severe problems at injection sites.

What Else Might Help

Many other nutrients help ease asthmatic reactions, and it is worth trying them in addition to strictly following the Anti-Inflammation Syndrome Diet Plan. People with asthma commonly have low levels of magnesium, a mineral involved in more than three hundred biochemical processes in the body. A high w-6 w-3 ratio, as is found in today's Western diets, promotes the pathogenesis of many chronic diseases, including cardiovascular disease, diabetes, asthma, and possibly cancer. Increased dietary intake of linoleic acid leads to oxidation of low-density lipoprotein (LDL), platelet aggregation, and interferes with the incorporation of essential fatty acids (EFA) in cell membrane phospholipids. Both w-6 and w-3 fatty acids influence gene expression. w-3 fatty acids have strong anti-inflammatory effects via the suppression of inflammatory cytokines IL-1, TNF-a, and IL-6. w-6 fatty acids tend to be proinflammatory. A few unfavorable metabolic effects of bBs were reported. bBs might exacerbate glucose intolerance 5,45,46 , increase triglycerides, and decrease high-density lipoproteins 47 . These effects on surrogate end points should not prevent us from using bBs in diabetic patients who have hypertension. Caution should be taken when using bBs in diabetic patients who are at risk for severe hypoglycemia, although there is no convincing evidence that b1-selective blockers increase the risk of masking hypoglycemia symptoms 48-50 . High dosages of selective b1-block-ers should be avoided in patients who have asthma or severe obstructive lung diseases. bBs should not be used in patients who have advanced heart block or sinus node disease without a pacemaker.

Types of Stress Tests

Adenosine and dipyridamole increase blood flow to nonarteriosclerotic vessels, unmasking stenoses in other arteries with their vasodilatory action. These agents should not be used if an individual has asthma or severe obstructive lung disease, because bronchospasm can be worsened.

Metformin and Repaglinide

A4 enzyme system, drugs that are metabolized through this system (Rifampin, barbiturates, carbamazepine, certain statin drugs, amiodarone, benzodiazepines, sildenafil (Viagra), theophylline, and certain selective serotonin reuptake inhibitors) may increase repaglinide metabolism (19). Although in vitro data indicate that repaglinide metabolism may be inhibited by antifungal agents (such as ketoconazole and miconazole) or antibacterial agents (such as clarithromycin), systematically acquired data is not available on increased or decreased plasma levels with other cytochrome P-450 3-A4 inhibitors or inducers.

Exubera Inhaled Insulin

Inhaled insulin is not a new insulin it is simply an old insulin (regular insulin) that is delivered into the body in a different way. You use an inhaler (very much like an asthma inhaler) to inhale the insulin powder, which is available in 1 mg and 3 mg blister packs. You should not use Exubera if you have lung disease such as asthma or bronchitis, or if you are a smoker. Smoking results in more of the insulin being absorbed, and you have to stop smoking for at least six months before you can start on inhaled insulin. There are higher levels of antibodies to insulin in the blood after using inhaled insulin compared to injected insulin, but this does not seem to have a negative effect. The cardinal feature of DKA is a deficiency of insulin action brought about by an absolute or relative lack of insulin (1,7,16). In newly diagnosed patients or when insulin therapy has been omitted, an absolute lack of insulin is responsible for the development of DKA (7,16). Inhaled Insulin

INH, glycaemic control or adverse outcomes (allergic events, pulmonary side effects or hypoglycaemia) 36,37 . Mild to moderate cough is seen with INH 34,36,37 . A major concern with INH is the potential for pulmonary toxicity because of the growth-promoting properties of insulin. Small decreases in lung function as judged by FEV1 and DLCO have been found to develop early in the treatment with INH but, however, it does not progress over 2 years. At present it can be concluded that INH offers an alternative non-invasive route for premeal insulin administration with glycaemic efficacy in T1DM 36,37 . Several studies report T1D incidence numbers of 0.1-36.8 100,000 subjects worldwide (2). When ACE inhibitors cannot be tolerated, e.g. owing to dry cough, then an angiotensin II receptor antagonist such as losartan, valsartan or irbesartan should be substituted. Beta-blockers are also helpful antihypertensive agents, especially post-myocardial infarct or when there is co-existing angina. They should be avoided in bronchial asthma or when there is severe peripheral vascular disease. Bendrofluazide at low dose (2.5mg day) effectively lowers blood pressure, but should be avoided in type 2 diabetes for fear of aggravating hyperglycemia. Indapamide may be a useful substitute. Calcium-channel blockers and a-receptor blockers, such as doxazosin are useful addon drugs to achieve satisfactory blood pressure control.


One of the most popular flavonoid supplements is also one of the most misunderstood. Quercetin (pronounced kwair-sih-tin), found in apples and onions, can inhibit the activity of some types of adhesion molecules, which promote many types of inflammatory reactions. Adhesion molecules enable white blood cells, which secrete pro-inflammatory compounds, to attach to normal cells. In terms of chemical structure, quercetin is similar to both rutin, another naturally occurring flavonoid, and to cromolyn sodium (NasalCrom), a synthetic flavonoid drug used to prevent pollen allergies.


The benefits of breastfeeding are discussed in Chap. 22 and include improved bonding, reduced maternal anxiety, monetary savings, decreased risk for postmenopausal hip fractures, and decreased infant illness with improved cognitive function of the newborn (28). Breastfeeding may lower the child's risk of developing diabetes and helps to prevent many infant and childhood illnesses such as otitis media, gastroenteritis, asthma, respiratory tract infection, obesity, and sudden infant death syndrome (SIDS) (28). Maternal weight loss is also a benefit. Calorie requirements are increased during lactation and are the same as calories needed in the third trimester with a recommended carbohydrate intake of 210 g per day (11). The estimated calorie expended in milk production is 500 kcal per day during the first 6 months and 400 kcal per day during the second 6 months of breastfeeding.


Accuracy of genotyping and exclusion of Mendel errors are important for success, but so is the careful definition of affection status. This may not always be easy for diseases like asthma, schizophrenia or systematic lupus erythematosus (SLE). Even with diabetes, the definition is based upon artificial cut-offs of plasma glucose. Dichotomising variables may result in loss of power. One alternative is therefore to search for linkage to a qualitative trait, e.g. blood glucose, blood pressure or body mass index, instead of diabetes, hypertension and obesity. Heritability (h2) is often used as a measure of the genetic component of a quantitative trait. The higher the heritability, the more likely it is a genetic cause of a trait will be found. Further consideration of demographic issues poses additional obstacles to generalizing from these results. In particular, two issues are very evident, the socio-economic status and ethnicity of participants. As Glasgow and Anderson163 have already noted, few researchers in this field publish data on recruitment rates and subsequent bias, but it seems reasonable to conclude that there is a bias towards white middle-class families. Furthermore, the vast majority of studies have been conducted in North America, further limiting the generalizability of studies. Finally, there is the issue of comorbidity, especially with concurrent additional chronic illness.
